Return the form, together with an initial payment of £1 either to a member of staff in one of our stores or to The Channel Islands Co-operative Society Limited, Registered Office: Co-operative House, 57 Don Street, St Helier, Jersey, JE2 4TR or Branch Office: The Channel Islands Co-operative Society, Nocq Road, St Sampson, Guernsey, GY2 4PB.

On acceptance, a share passbook bearing your personal share number will be posted to you (usually within 2 weeks from receipt of application) and will indicate your holding of a minimum £1.00 share. After 6 months provisional membership, during which time you still earn dividend, you become a full member of the Society and you are entitled to all the benefits of such membership. You will also have the right to vote at the Society's Annual General Meeting.
